New patients will need to fill out two forms. The first covers general information and consent for treatment and for HIPAA consent. The full privacy statement is also available here. The second form is a health history form. Both the registration and health history forms should be brought to the office on the first office visit. If you are an established patient and there are changes in address, insurance or other information, then you may submit an updated registration form on your follow up visit. |
